To facilitate processing the paperwork for U.S. immigration physical exams, please bring the following required documents:

1. Picture identification (required). (i.e. Driver’s License, State ID, Passport, Work Permit, School ID, or any other Government issued photo ID).
2. Marriage License if applicable.
3. Vaccine/Shot records if available.
4. Alien Registration number if you have one (starts with an “A”).
5. Please call for pricing and scheduling an appointment. We accept cash, credit cards, money orders, cashiers checks. No personal checks accepted.
