To facilitate processing the paperwork for Canadian immigration physical exams please bring the following required documents:

1. A government issued picture Identification. (Passport)
2. Canadian Immigration File Number (if available).
3. Six (6) passport type photographs if sponsored, Four (4) passport type photographs if not sponsored. Must be taken within six months. Required for all applicants.
4. If you have the forms, please bring them, otherwise we will provide the required forms.
5. Please call for pricing and scheduling an appointment. We accept cash, credit cards, money orders, cashiers checks. No personal checks accepted.
